Affidavit of Identity and Receipt of Filing
This form is used as part of the process in running for an elected office. The candidate must complete the form which declares residency, party affiliation, if required, office for which the candidate is running, the term of the office, and how the name is to be placed on the ballot.
Affidavit of Write-in Candidate
This affidavit is completed by persons who wish to run as a Write-in Candidate in a given election.
Application for Special Election
The Application for a Special Election is used by a taxing unit when a special election is needed to levy additional millage, present a referendum to the electorate, etc. A representative from the taxing unit must complete the application indicating purpose of the election, the units of government to be affected, and a proposed date.
Precinct Delegate Affidavit of Identity and Receipt of Filing
This form may be filed with candidate’s county clerk, or, as an alternative, the candidate’s city or township clerk. A city or township clerk receiving this form must forward a copy to the county clerk within four days after the filing deadline.
